Please return the completed form to our office staff or post to Central Medical, PO Box 140, Oamaru 9444. Alternatively you can scan and email it to us at admin@centralmedicaloamaru.co.nz
Alternatively, please call into see our office staff who can provide you with a form and assist you in completing it.
Privacy Statement
We collect personal information from you, including information about your:
- name
- date of birth
- address
- contact information
- billing or purchase information
- previous healthcare provider
We collect your personal information in order to:
- provide appropriate health care
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. We keep the personal information we hold safe by storing it in encrypted files and only allowing certain staff to access it.
We keep people’s personal information for two years, at which point we securely destroy it which includes securely erasing all digital copies.
